Residency Affidavits
All students in the Alvin Independent School District under a residency affidavit must have their residency verified annually to avoid withdrawal from the Alvin Independent School District.
** If you no longer require a residency affidavit, please deliver copies of the required proof of residency documents (mortgage, lease, and utility bill in your name) to the school to correct their records.
Please be advised all affidavits are:
One per student
Both the resident and parent/guardian are required to submit affidavits, in person, to the Student and Administrative Services Department or the Central Enrollment Center. Notary available at both sites.
Required to show the supporting documents as listed on each packet
Required to be approved by Student and Administrative Services

Parent’s/Guardian’s Assurance of Bona Fide Residency Affidavit
This form is used if the parent does not have a lease or mortgage statement in their name and resides with someone in the Alvin Independent School District attendance zone. The attachments required along with the completed affidavit are as follows:
Parent - Government Issued Photo I.D. (address must be current)
Parent - Current mail showing resident's address
Student’s Birth Cert. or other acceptable proof of identity
Resident - Government Issued Photo I.D.(address must be current)
Resident - Current Utility Bill (Gas, Water or Electric – Disconnect Notices will not be accepted)
Resident - Current Lease or Mortgage Statement (Property Tax Statement may be substituted)
Only one student per original affidavit
Grandparent’s After-School Care Affidavit for Non-Resident Student
The Grandparent Affidavit may be used only if the parent lives outside the AISD attendance zone and the grandparent lives within the AISD attendance zone. The attachments required along with the completed Affidavit are as follows:
Parent’s Government Issued Photo I.D.
Parent's Current Utility Bill (Gas, Water or Electric – Disconnect Notices will not be accepted)
Student’s Birth Cert. or other acceptable proof of identity
Grandparent's Government Issued Photo I.D.
Grandparent's Current Utility Bill (Gas, Water or Electric – Disconnect Notices will not be accepted)
Grandparent's Current Lease or Mortgage Statement (Tax statement may be substituted)
Copy of the Parent’s Lease or Mortgage statement to verify they reside outside the attendance zone.
Only one student per original affidavit.
Guardianship Notification
This affidavit is used if the parent is giving education rights to a person other than the parent (i.e. family, friend or neighbor), due to the student residing with someone other than a parent.
*This Affidavit does not take away from the parental rights in any way, but only grants educational rights to the guardian and gives the campus permission to discuss the student with the guardian.
The attachments required along with the completed Affidavit are as follows:

Student's current discipline and attendance record
Guardian’s Government Issued Photo I.D.
Guardian’s Current Utility Bill (Gas, Water, or Electric – Disconnect Notices will not be accepted)
Guardian’s Current Lease or Mortgage Statement (Tax statement may be substituted)
